FUNCTION OF WORK:To provide advanced level implementation, coordination, monitoring and evaluation of programs within offices specializing in services for those with developmental disabilities.
LEVEL OF WORK:SUPERVISION RECEIVED:Broad from a Community Services Supervisor, a higher-level professional or an administrator.
SUPERVISION EXERCISED:May provide functional supervision over lower level Community Services Professionals.
JOB DISTINCTIONS:Differs from the Community Services Professional 2 by the broad supervision received and the presence of more independent and complex assignments.
Differs from the Community Services Supervisor by the absence of direct-line supervision of professional and support level staff.

Assists clients in developing service plans including identification of goals and appropriate services, making recommendations for specific support needs.
Conducts complex assessments of clients utilizing standardized assessment tools.
Meets with officials of local and state agencies, professional organizations, advocacy organizations and private providers to develop and explore resources/service delivery for individuals.
Prepares appeals packages for submission to the Bureau of Appeals and participates in appeals hearings by presenting evidence for agency decision.
Prepares reports, maintains records, and conducts correspondence relevant to area of specialty.
May provide functional supervision and training to less experienced staff in selected areas of expertise.
Interviews and establishes working relationships with clients and families.
Serves as a liaison with a variety of local and state agencies, professional organizations, advocacy organizations, legal and court related entities, and private providers to develop, maintain, and enhance a viable service delivery system for individuals with developmental disabilities and their families.
Plans and coordinates a portion of a statewide program element in a specific service area.
Conducts educational presentations or in-service training for community groups, agencies, providers, etc. on developmentally disabled eligibility and programs.
Provides supportive services for clients and assists in obtaining services from appropriate community resources based on the support plan.
ENTRY UNIT:
Assesses home environment, living situation, and the abilities of clients and families to determine the need for support services; also determines if other agency referrals are necessary for crisis intervention.
Completes standardized assessments for determining eligibility of developmentally disabled state and federally/mandated programs and services.
Determines the current status of clients to evaluate if an immediate danger/risk is present.
Obtains and/or researches medical, psychological, educational reports about clients; determines if information supports eligibility requirements as established by state law.
Prepares eligibility decision case file.
FAMILY SUPPORT UNIT:
Implements the developed service plan and monitors compliance by providers, family members and others to ensure the supports are delivered satisfactorily.
Participates in setting case plan goals/objectives in family team conferences and presenting requests for funding as appropriate within an agency staffing meeting.
Monitors the flow of required documents for fiscal oversight of service plan.
Negotiates terms for complex social services contracts through direct personal contact with administrative provider agency personnel.
Performs on-site interviews with provider agency administrators and assure program compliance with contract expectations.
Coordinates exploration of alternate living arrangements for clients/families requesting such, as outlined in plan of services.
QUALIFICATION REQUIREMENTS:MINIMUM QUALIFICATIONS:
Six years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR
Six years of full-time work experience plus three years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR
A bachelor's degree plus three years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR
An advanced degree in a social sciences field, nursing, public health; public, hospital, or health administration; speech-language pathology or communication sciences and disorders; physical, occupational, recreational, or music therapy plus two years of experience in providing health or developmentally disabled services, counseling, health care management, or social services.
EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.
